Definition of Complete Denture
A fixed or removable prosthesis that replaces the entire dentition and associated hard and soft structures of the Maxilla and Mandible.
This procedure includes the restoration of the major function lost which are oral functions, comfort, and restores the appearance of patient.
AIM & OBJECTIVES OF COMPLETE DENTURE
1)Replacing lost natural teeth with artificial teeth. These teeth must be placed withen confines of functional balance of musculature which are involved in controlling food bolous between the occlusal surfaces of teeth. This will restore mastication.
2)Phonetics: Complete Denture enhances the speech and phonetics quality of the patient because of correct jaw relation and ability of the denture to provide various dento labial sounds.
3) Esthetics: Complete Denture enhances many folds the appearance of the edentulous patient.The visibility of custom made artificial dentition which can be characterised blends with the individual looks. Parallelism of jaw,prognathic appearance,deep nasio-labial fold,loss of labiodental angle,narrow lips,increase columella-philtra angle,loss of labiodental angle can be reversed considerably to give a haromnious & pleasant apperance to the patient.

The fabrication of Complete Denture is done in two parallal phases Clinical and Laboratory steps.
Clinical step initial focus will be on evaluating the patient and plan a treatment mode. The most important step here is analysing the Mental health of the geriatric patients.
M.M.HOUSE classification makes it easy to plan and assess the prognosis of Complete Denture.
1)Philosophical: patient has no past denture experience. They are easy going.Adjust with dentist and co operate.
2) Exacting:Precise,above average in intelligence.Usually dissatisfied by previous experience. Difficult to satisfy but once convinced then loyal for life.
3)Hysterical: Negative attitude , don’t want treatment,go to Dentist on compulsion of family members.Most difficult to treat.
4)Indifferent: Unconcerned about appearance,oral and personal health.Uncooperative towards Dentist and attempt to wear denture Do not appriciate Dentist work.
Intra Oral Examination
Maxillary and Mandible edentulous ridge can be broadly classified based on condition of hard and soft tissue remains.
A) Minimum ridge resorption. Firm healthy soft tissue.With hamular nothch well defined in maxilla and adequate tongue space in mandible. Good prognosis.
B)Sufficient residual ridge but undercut present surgical procedure may or maynot be done.
C)In maxilla anterior resorbtion takes place employ modified impression technique. In mandible same as B but surgical treatment is required.
D)Resorption of bone results in hyperplastic soft tissue.
E)Hyperplastic tissue may require surgical treatment.
F)Residual resorbed ridge compromise retention and stability.Requires ridge augmentation procedure,vestibuloplasty,bone graft.
PALATAL VAULT: Varies in patients favorable vault form is that has medium depth with well defined rugae area in anterior part. Flat vault is mostly associated with anterior bone resorption. It cannot resist lateral or anterior force.Deep palatal vault generates excess pressure on hard palate results in loosening of denture.
BIOLOGICAL CONSIDERATION
Both arches have 2 common structures,Limiting structures and Supporting structures.
MAXILLARY LIMITING STRUCTURES: Labial frenum,buccal frenum,buccal vestibule,pterygomaxillary notch,fovea palatine,Vibrating line.
MAXILLARY SUPPORTING STRUCTURES:Residual ridge,incisive pappila,posterior palatine area,bone of basal seat,incisive foramen,Maxillary tuberosity.
MANDIBULAR LIMITING STRUCTURES :Labial vestibule,buccal frenum,buccal vestibule,external oblique ridge and buccal flange,Massetric muscle region,Distal extensions limiting structures are Ramus of mandible,Buccinator muscle that crosses from buccal to lingual as it attaches to pterygomandibular raphe and superior constrictor muscle.
Retromolar region and pad is bounded anteriorly by border of ramus,it contains glandular tissues,fibers of temporal tendon,buccal fibres of buccinator muscle,fibers of superior pharengeal constrictor muscle from lingual side.The pterygomandibular raphae enters the pad at its top back inside corner.
Alveololingual Sulcus:Anterior region extends from lingual frenum to place where mylohyoid ridge curves down below level of sulcus.
Middle Region:From premylohyoid fossa to distal end of mylohyoid ridge.The sulcus curves medially towards mandible which is caused by mylohyoid ridge.
